Layout Help!

I've been playing around with my layout for an hour or two now and I'm still not happy with it. The layout pic hasn't been sitting properly and it keeps getting cut off. So, yeah. Basically it's driving me insane. Anyone want to help out?

What I've been trying to do is insert the following header into a layout, "Expressive", with the theme, "Icicles" if that makes sense. If you know how to do that, please comment! I'll write you a fic or something! *begs*

Oh, and also, I can't remember where I got the layout header from. If you know who made it, let me know so I can credit them too please. Thanks!
